Outreach and Support Worker - Fareham
We have an exciting opportunity for Outreach and Support staff to join our team providing highly individualised flexible support to people with disabilities or health needs living in their own homes or in a Supported Living home.
The hours of work are varied and flexible dependent upon the needs of the client & the work which is available. We are looking for full time and part time staff.
The role is based on a non-guaranteed hours contract.
Shifts may include short domiciliary visits which start from 1 hour extending to 12.5 hours in length, sleep-over visits & Waking Night Duty.
To provide highly individualised flexible support to people with disabilities or health needs living in their own homes or in a Supported Living home.
To respect the lifestyle and choices of clients to ensure they lead fulfilling lives, physically, intellectually, emotionally and socially, whilst working in accordance with CMGs philosophy of care and values, its Policies and Procedures and all relevant legislation.
To assist with daily living activities specified within the client Care plan.
To support clients to take part in social or community events as required
To monitor the well being of clients
The Ideal Candidate
You must be a car driver, have your own car and live in the Fareham/Stubbington/Gosport area.
Experience is desirable but not essential as we provide comprehensive training and support.
Basic numeracy and literacy skills are a must as well as good spoken English.
We operate the Government's childcare vouchers initiative, which are taken from your salary before your usual Tax and NI contributions are deducted, and also the Government's pension scheme.
We offer all staff 28 days annual leave, rising to 30 days after 1 year,with an option to buy up to 5 days additional holiday per year. Annual leave and additional holiday are prorated for part time staff.
